On more than one occasion, I've been criticized for "bashing GLP-1s." The accusation stings a little bit, but only because it misses the point of everything I've tried to communicate over the past fifteen years. The truth is, I get it. I understand the appeal. When you've struggled with your weight for years—when you've tried every diet, joined every gym, downloaded every app, and still found yourself unable to maintain results—the promise of a weekly injection that simply makes the hunger go away must feel like salvation.


I'm not here to take that hope away from you. I'm here to ask you a question.


Is it worth it?


The Risk-Reward Calculation


Every medical intervention involves a calculation. On one side, you have the potential benefits. On the other, the potential risks. For someone who is dangerously obese—whose elevated glucose and HbA1c levels signal immediate metabolic crisis, or who has already crossed the threshold into Type 2 diabetes—the calculation tilts toward GLP-1 medications. The questionable side effects, and let's be honest, the unknown long-term consequences that we won't fully understand for another decade, are outweighed by the immediate risk that their health is already in.


These are the conditions GLP-1 receptor agonists were designed to treat. They emerged from diabetes research, developed specifically to improve glycemic control in patients whose bodies had lost the ability to regulate blood sugar effectively. For that population, the benefits are clear and the risks are acceptable.


But what if you're not in that population?


What if you need to lose thirty pounds? Forty? What if you put on weight over the winter, or during a stressful year at work, or after having kids? What if you're frustrated with your appearance but not in immediate medical danger?


Is it wise to subject yourself to a pharmaceutical intervention with unknown long-term effects—effects on your metabolism, your neurological system, your capacity for pleasure—when there's a safer path available?


GLP-1s act on your pancreas, your hypothalamus, your brainstem, and the dopamine pathways that govern reward and motivation. They don't just reduce your appetite. Execution Is the Easy Part


I've said this before, and I'll keep saying it until it sinks in: execution is the easy part.

If you know what to do, all you have to do is follow the rules. The hard part was never the doing—it was the deciding. It was the confusion, the conflicting advice, the paralysis of not knowing which approach would actually work for your body.
